A Guide From David

Using a Superbill

My practice is private pay, and I can provide a superbill on request if you want to seek out-of-network reimbursement. Your insurance plan determines whether it will reimburse you and what requirements apply.

Ask Your Plan Before Relying on Reimbursement

Contact your insurer to ask whether your plan includes out-of-network outpatient mental health benefits. Ask about a deductible, any authorization requirements and how claims should be submitted. A benefit summary may not answer every question about your particular coverage.

Understand the Role of the Superbill

A superbill provides information an insurer may request when reviewing a claim. It is not a promise of reimbursement or an agreement that I bill the insurer directly. We can discuss what information would be included so you can make an informed choice about submitting it.

Keep Payment and Reimbursement Separate

My fees are $190 for a 50-minute session and $110 for a 30-minute session. Couples and family sessions cost the same as individual sessions. Payment is due on the day of, or the day after a session unless we arrange otherwise. Any decision by your insurer is separate from that payment arrangement.

Bring Questions Before You Begin

If the cost of therapy depends on reimbursement for you, raise that before committing to a schedule. You can ask me about the documentation I provide and ask your insurer about its decision process. I cannot promise how much your plan will pay or whether a claim will be accepted.

A Question to Bring With You

What would you need to know about your plan and your own costs before deciding on a therapy schedule?

Will a superbill guarantee that my plan pays?

No. Coverage and reimbursement are decisions made by your insurance plan.
